This study addresses the clinical need for uncapped dosing in obesity-related VTE risk, highlighting a significant dosing gap. Analysis of 219 patients revealed that 89% had anti-Xa levels below the therapeutic threshold, with minimal association to adverse outcomes. Recommendations suggest that uncapped dosing at 200 IU/kg is effective, with routine monitoring potentially unnecessary for patients with Class I–II obesity, thus optimising treatment strategies.
